Cryobiopsy Under Local Anesthesia Confirms Mesothelioma

New Method Provides Quick Mesothelioma Diagnosis Despite Patient’s High Surgical Risk

According to the recent mesothelioma case study published in the journal Respirology Case Reports, a 65-year-old construction worker with a history of asbestos exposure presented with chest pain and a 22-pound weight loss over three months. These common warning signs of mesothelioma warranted a CT scan, which revealed diffuse irregular thickening of the left pleural lining. This further raised suspicion of Stage III mesothelioma, but extensive calcified plaques and severely narrowed spaces between the ribs suggested that scar tissue had adhered the left lung to the chest wall, ruling out the thoracoscopy that would normally have been the next diagnostic procedure.

The patient’s overall condition compounded the diagnostic challenge. His lungs were severely restricted, and he had experienced recent rapid functional decline requiring use of a cane for walking. These factors made general anesthesia a high risk, but without a tissue sample, a diagnosis of mesothelioma couldn’t be confirmed, and treatment couldn’t begin.

Approach Offers New Tool for Diagnosing Mesothelioma Without General Anesthesia

The team from the Tokyo Metropolitan Cancer and Infectious Disease Center solved its diagnostic dilemma by using CT to map the tumor and ultrasound to confirm adhesion patterns. This allowed them to make a small skin incision directly over the tumor, perform minimal blunt dissection of adhesions, and introduce a device called a cryoprobe that uses extreme cold to freeze and extract tissue through the opening. Freezing times of 10 to 14 seconds produced a tissue specimen of excellent quality that confirmed epithelial mesothelioma.

The mesothelioma diagnostic procedure lasted 58 minutes, including a 24-minute wait for frozen section analysis, and the patient was walking just two hours afterward and was discharged the following day. Immunotherapy with ipilimumab and nivolumab began 22 days after the procedure, and the patient had been progression-free for eight months at the time of publication. The authors concluded that cryobiopsy under local anesthesia may be feasible for mesothelioma patients with severe pleural adhesions when safe access to the pleural space is achievable and sufficient target tissue can be obtained.
